If you’ve ever felt rushed, dismissed, or like no one is really listening, Renew Family Medicine & Hormone Therapy was created with you in mind. We’re accepting new patients ages 6 and up, including teens, adults, midlife adults, and seniors.

This page will walk you through how to get started, what to expect, and what you need to bring so your first visit feels calm, clear, and supported.

Step 1: Request an Appointment

You’ll typically start in one of Three ways:

  • By text — text our office number to request an appointment or ask brief scheduling questions
  • Online – complete a short “Become a Patient” request form through our website or patient portal
  • By phone – call our office to schedule a new patient visit
Texting should only be used for scheduling and registration questions. Medical questions must be handled by phone or portal for safety.
Texting is not monitored 24/7 and should not be used for urgent concerns.

 

Step 2: Complete New Patient Forms

Before your first visit, we’ll ask you to complete:

  • New patient registration
  • Medical history
  • Medication & allergy list
  • Consent forms 
  • Any relevant release of records so we can request past records if needed 

You can complete these online in advance. If you prefer paper, we can arrange that too.

Step 3: Send Prior Records

We may ask you to fill out a Release of Record Form if you have

  • Recent labs or imaging
  • A history of chronic conditions (e.g., heart disease, thyroid, diabetes, autoimmune)

For ADHD medication management  we require:

  • Documentation of prior ADHD diagnosis

  • Documentation of prior treatment (medications, evaluations, or specialist notes)

We’re happy to help guide what to request if you’re unsure.

Option 2: Direct & Empowering

Step 4: Your First Visit – What to Expect

No 15-minute rush jobs here. Your first visit is a real conversation where you're actually heard.

What we'll do:

  • Deep dive into your health history and current concerns
  • Talk about your goals—energy, sleep, hormones, heart health, weight, mood, and beyond
  • Review all medications and supplements
  • Complete vitals and a focused physical exam based on your unique needs

You ask questions. We answer them. Together, we create a plan that actually works for your life.

Telehealth Availability

Telehealth may be an option for:

  • Follow-ups
  • Medication management visits
  • Lab review
  • Some mental & emotional health visits

We’ll let you know when telehealth is suitable and when an in-person visit is safer or required.

Accessibility & Comfort

We want visits to feel as low-stress as possible, especially for:

  • Patients with anxiety
  • Neurodivergent patients
  • Patients with prior medical trauma
  • Older adults and caregivers

You’re welcome to:

  • Bring a support person to your visit
  • Write down questions ahead of time
  • Ask for clarification or repetition
  • Take notes during the visit

You will not be rushed for asking questions.
